MEDICAL EXAMINING BOARD
FRIDAY, DECEMBER 21, 2012
A Meeting of the Medical Examining Board was held on Friday, December 21, 2012, at the Office of the State Comptroller, 55 Elm Street, Room 208, Hartford, Connecticut, 06106.

Please note that a portion of the meeting was conducted in executive session as the focus of the meeting was the medical condition of each Disability Retirement applicant.
CALL TO ORDER
The Chairperson called meeting to order at 9:38 am.
Dr. Samson motioned to go into the Executive Session to discuss medical
condition(s) related to the applications for Disability Retirement, seconded by
Dr. Sirop at 9:39 am. Members of the Retirement Services Division were asked to
be allowed to remain.
Dr. Samson motioned to come out of Executive Session, seconded by Dr. Sirop
at 12:19 pm.

Dr. Kamath moved, seconded by Dr. Sirop, to recommend denial of Matthew
Campbell'S
application for Disability Retirement. Motion approved unanimously.

Dr. Kamath moved, seconded by Dr. Sirop, to deny Elona Chubbuck'S
entitlement
for Service-Connected Disability Retirement. Motion approved unanimously.

Dr. Kamath moved, seconded by Dr. Sirop, to stand by the decision made
previously by the Board that Victor Diaz was not entitled to Service-Connected
Disability Retirement. Motion approved unanimously.
