ATTENTION: Personnel and Payroll Officers
SUBJECT: Tier IIA Summary Plan Descriptions

The Tier IIA Summary Plan Descriptions ("the booklets") are scheduled to be mailed to the home addresses of Tier IIA members of the State Employees Retirement System (SERS) on or about August 1, 1999. This mailing will include Tier IIA members hired from July 1, 1997 through May 6, 1999 provided their Form CO-931 was received by this Division by May 31, 1999. For agency use a copy of this booklet accompanies this memorandum.

With employee paychecks dated August 13, 1999, the Division will advise Tier IIA members of the aforementioned mailing, and in the event that their booklets do not arrive, instruct them to contact their personnel/payroll office. For your information, the text of the paycheck insert appears on the reverse side of this memorandum.

The booklets are included under the Forms Management Contract with Vanguard Direct and will be printed on an as needed basis. Agencies may order booklets from Vanguard for Tier IIA members in service on or after May 7, 1999 and for those Tier IIA members in service prior to that date who advise that they did not receive the mailed booklet. A limited number of booklets are currently in stock at Vanguard; therefore, on initial orders agencies will be limited to a maximum of twenty (20) booklets. Thereafter agencies may order booklets in groups of ten (10) to fulfill their ongoing processing needs. Agencies should write in "CO-201, Retirement Tier IIA Booklets" in the section entitled "other" on the Statewide Forms Requisition when placing the orders. As you are aware, each SERS member is entitled to receive a summary plan description within 90 days of employment.

It is anticipated that the revised booklets for Tier I and Tier II members will be available before the end of this calendar year. Another memorandum will be provided to agencies with information regarding the Tier I and Tier II booklet mailing dates and ordering procedures.

For Tier IIA members hired between July 1, 1997 and May 6, 1999, a summary plan description ("the booklet") was mailed to your home address on or about August 1, 1999. If you did not receive your booklet or you began your Tier IIA membership after May 6, 1999, please obtain your copy by contacting your agency's personnel/payroll office.
