As of December 31, 2010, Aflac New York will no longer receive premium deductions through your employer, The State of Connecticut . Aflac New York has provided benefits to The State of Connecticut employees since October 1, 2001, and we are pleased to give you the good news that you can continue your coverage directly with Aflac New York at the same payroll rate.
In January 2011, you will receive correspondence offering continuation of coverage on a direct billing basis. Simply choose how often you would like to pay your premiums and return the letter to us with a check or money order. Once your policy is transferred to direct billing, we will mail your premium notices approximately one month before each premium due date. Although payment is due on that date, you have a 31-day grace period after the premium due date before your policy terminates due to nonpayment.
Many of our policyholders like to use our easy and convenient draft plan to pay their premiums. This plan allows your financial institution to automatically draft your account each month on the designated draft date and send your premium directly to us. If you select this method of payment, additional instructions will be provided with the next letter.
