| SUBJECT: | OSC - Retirement Services Division Forms |
The Retirement Services Division is in the midst of implementing the Pension Module which will better serve active employees and retirees while staying current with the ever-changing state and federal regulatory requirements. Due to changes in the processes of the Retirement Services Division all of our forms have been revised and are now available on the Office of the State Comptroller website at http://osc.ct.gov/rbsd/retirement/forms.html . All of the forms have been updated to include a separate name block for scanning purposes. However several forms have been updated and redesigned.
Please note if a State agency has redesigned a Comptroller's Office form for specific use within that agency the Retirement Services Division will no longer be able to accept the form. In addition, as of January 4, 2016 the Retirement Services Division will be unable to accept any older versions of Retirement Services Division forms.
The Division welcomes your written suggestions as to the format or wording of our forms. Please submit your suggestions to our central email address osc.rsd@ct.gov.
