The purpose of this memorandum is to announce that the opportunity for active employees to apply for long term care insurance under Transamerica's simplified underwriting standards will end April 30, 2012.

As a new vendor with the State in 2011, Transamerica offered an extended "open enrollment" period, allowing active employees between the ages of 18 and 65 to apply for long term care insurance by answering only nine health-related questions. After simplified underwriting ends on April 30th, applicants seeking long term care insurance will need to respond to a more detailed set of twenty-eight questions about their health to obtain coverage.

Regardless of the underwriting standards applied, the discounts, products and services offered by Transamerica will remain the same.

State of Connecticut employees and retirees who are interested in obtaining information about long term care insurance should contact Transamerica at 1-800-582-2889.
