| Subject: | Statewide Forms CO-860, CO-790, and CO-790S |
I. PURPOSE
The purpose of this memo is to advise all State agencies that the following Comptroller forms have been revised and can be downloaded from the Comptroller's website:
| CO-860 | Request for Payment Information |
| CO-790 | Payment Cancellation |
| CO-790S | Payment Cancellation Statement |
II. PROCEDURES
The revised forms are in Microsoft Excel 2003 format and available from the forms section of the Comptroller's website at http://www.osc.state.ct.us/agencies/forms/. Although the forms can be completed on-line, an original written signature is still required. The completed form will have to be printed, signed, and submitted to the appropriate office. For those agencies that do not have Internet access, you may contact the Office of the State Comptroller, at the telephone number listed below to obtain forms. Accompanying this memo are copy-ready samples of the forms.
