| Subject: | Income Payment Election Forms |
The purpose of this memo is to advise all State agencies that the following forms under the Forms Management Contract with Vanguard Direct are obsolete:
CO-899 Income Payment Election - Option "A" REV. 7/87
CO-902 Income Payment Election - Option "D" -REV. 6/95
All Income Payment Election forms in use should be dated NEW 9/82. Agencies should discard all other copies and only use the correct forms dated NEW 9/82.
The reverse side and attached page of this memo contains a sample of the correct forms. These forms are available through Vanguard Direct at 860-563-1054.
