The Subcommittee met on this date to consider the cases noted below.

Unanimous Recommendations

(1). Elizabeth Bayona The Subcommittee unanimously recommends denial of this application due to the absence of a Personal Service Agreement (PSA) which would validate her employment.

(2). William Congero The Subcommittee unanimously recommends approval of Mr. Congero's request that he receive a partial refund of the interest cost associated with his membership transfer from Tier II to Tier I and purchase of prior service.

(3). Nanci Mae Gminski The Subcommittee unanimously recommends denial Ms Gminski's request for a refund of her retirement contributions as a vested rights member of the SERS Tier IIA retirement plan.

(4). Beverly Herbert The Subcommittee unanimously moved to deny Ms Herbert's request that the Retirement Service Division retroactively change her effective date of retirement from June 1, 2010 to January 1, 2010.

(5). Dyanna Hines The Subcommittee unanimously moved to deny Ms Hines request to change her type of retirement from voluntary to disability subsequent to her receipt of a retirement pension.

(6). Nicholas Iannarone The Subcommittee unanimously recommends denial of the request of Stella Iannarone, to change the straight life annuity option election of her late husband Nicholas Iannarone to an option election that would provide for a continuation of his SERS benefit.

(7). Rush Johnson The Subcommittee unanimously recommends denial of Ms. Black's request that a posthumous non-service connected disability retirement application for her late brother be made electing the Ten Year Period Certain option.

(8). Kevin Mullane The Subcommittee unanimously recommends approval of a June 1, 2011 date of retirement under Connecticut General Statutes, Section 5-163(c) for Mr. Mullane and denial of the requested February 1, 2011 retirement date.

(9). Michael Perrault The Subcommittee unanimously recommends approval of the request for a posthumous disability retirement application to be made for Mr. Perrault with the election of the fifty percent to contingent option for his daughter.

(10). Roberta Prymula The Subcommittee unanimously recommends denial of Ms. Prymula's request to purchase retirement credit in Tier IIA for her prior service under a contractual arrangement from February 2000 to August 2000.

(11). Alba Rodriguez The Subcommittee unanimously recommends approval of Ms. Rodriguez's request to change her retirement payment option selection from Option B 100% (naming her son as contingent annuitant) to Option D Straight Life subsequent to her receipt of the retirement benefit.

(12). George Rumney The Subcommittee unanimously recommends denial of the request of Holly Chase Rumney to change the straight life annuity option election of her late husband George Rumney to an option election that would provide for a continuation of his SERS benefit.

(13). Jeffrey Surprenant The Subcommittee unanimously recommends denial of Mr. Surprenant's request to obtain partial retirement credit for his period of military leave time at no cost.

(14). Gail Treske The Subcommittee unanimously recommends denial of Ms. Treske's request for a reconfiguration of her SERS Tier I retirement benefit as she had recently attained age fifty-five.

(15). Frederick Gilbert: The Subcommittee directed that this matter be tabled until the Division is in receipt of notarized statements from the individuals who were identified as witnessing Mr. Gilbert's signature on certain retirement forms with the intention of retiring with a disability retirement.

(16). Patricia Neafsey: The Subcommittee directed that this matter be tabled citing the need for further information.
