RE:	Kaiser Foundation Health Plan - Advisory

The Office of the State Comptroller is mailing letters dated October 27, 1999 to members of Kaiser Foundation Health Plan of Connecticut, Inc (Kaiser) notifying them that effective November 1, 1999, ConnectiCare, Inc will be providing the health care coverage formerly provided by Kaiser.

To the extent that this letter generates questions concerning this matter, kindly advise inquiring employees that:

ConnectiCare will be mailing informational membership kits, which will provide all the information needed to access health care through ConnectiCare's network including their membership cards by November 1, 1999.
Shortly thereafter, provider directories will be mailed to the members and to agency payroll offices.
If employee's do not receive their membership kit or if you or your employee's have additional questions regarding benefits or provider participation, please call the dedicated State of Connecticut number at ConnectiCare, 1-800-727-3712.

Please be advised that you do not have to take any action. ConnectiCare is working directly with this office to insure a smooth transition.
