COMPTROLLER'S MEMORANDUM NO. 97 - 61

TO THE HEADS OF ALL STATE AGENCIES

ATTENTION: Personnel and Payroll Officers
SUBJECT:State Employee Group Life Insurance Plan

I. INTRODUCTION

The purpose of this memorandum is to (1) advise that the Office of the State Comptroller has procured a new policy agreement for the state employee group life insurance plan, and (2) address administrative issues relative to the new arrangement.

II. GROUP LIFE INSURANCE PROVIDER

The previous group life insurance policy between the Office of the State Comptroller and Aetna US Healthcare terminated on June 30, 1997. Medical Life Insurance Company (MLI) has been selected as the successor provider and a new policy agreement between the Office of the State Comptroller and MLI went into effect July 1, 1997.

III. ADMINISTRATIVE ISSUES

From an administrative perspective, the change in providers has been essentially a seamless transition in that the policy provisions between the predecessor and successor agreements have not been altered and the processing procedures remain unchanged. MLI has prepared new group life insurance booklets describing the benefits of the group life insurance plan and provided new processing forms for use with the new life insurance policy. Booklets and forms are expected to be available to the agencies in January 1998.

Agencies must obtain three forms and group life insurance booklets through Corporate Express using the appropriate Corporate Express Order Number identified below. Agencies should order the forms in quantities sufficient for their operating needs; group life insurance booklets should be ordered in a quantity sufficient enough to distribute one to every employee and to maintain a supply for new employee orientation materials. There is no charge for these items and they will be delivered with the regular office supply orders. Any need for group life insurance forms which are not available through Corporate Express may be obtained by contacting the Employee Benefits Unit of the Comptroller's Retirement & Benefit Services Division at (860) 702-3542.

Once agencies have obtained the group life insurance booklets, a copy should be distributed to each employee along with notification of the change in policy providers.

IV. CONCLUSION

Questions related to the group life insurance policy should be directed to the Employee Benefits Unit at the number provided herein. Your assistance in notifying your agency's employees that MLI is the new policy provider and in distributing new group life insurance booklets is appreciated.
