MEMORANDUM NO. 95 - 13

TO THE HEADS OF ALL STATE AGENCIES

Attention:Chief Administrative and Fiscal Officers, Business
Managers, and Payroll and Personnel Officers
Subject: Canceled Payroll Checks, Advance Vacation Payments,
and Other Recoveries

Effective April 1, 1995, the following new procedures will be implemented for submitting canceled payroll checks and various recoveries including; Workers' Compensation, employee overpayments, and advance vacation payments.

  1. Form CO-1081, Agency Multi-Purpose Payroll Report, will replace forms CO-678, Detailed Analysis and CO-19, Expenditure Batch Header for the recording of taxes and deduction recoveries as well as for advance vacation payments. Forms CO-19 and CO-678 will no longer be accepted after the above effective date.

  2. Form CO-1081 must be properly completed as to full coding and signature requirement. The white and yellow copies must be sent to Central Payroll Division and the blue copy kept by the agency. Form CO-1081 can be ordered from the current supplier, Moore Business Forms & Systems Division, 100 Northfield Drive, Windsor, CT 06095, Telephone (203) 298-1500 or FAX (203) 688-7717, using Stock Number 6938-706-01. Care must be taken to assure that debit and credit amounts are entered in the proper columns. Attached is a sample of CO-1081 properly filled out.

  3. All other procedures remain intact.
