Instructions

Submit copies of the financial statements (balance sheet and operating statement) which your agency may prepare as of June 30 for the following fund types: activity, enterprise, internal service and welfare.
Submit additional financial information according to attached form, if your agency is required to do it.

If your agency does not prepare any such financial statements and is not required to submit any additional financial information, please check "form not applicable" on GAAP Forms Control Sheet.
