ATTENTION: All Human Resources and Payroll Officers
SUBJECT: New Form CO-898a, "Application for Alternate Retirement Program Retirement Benefits"

To clarify and streamline procedures for both State agency and Retirement Services Division (Division) staff, a new form specifically for Alternate Retirement Program (ARP) members to apply for retirement benefits has been created; Form CO-898a, "Application for Alternate Retirement Program Retirement Benefits".

The new form should be used immediately in lieu of Form CO-898, "Application for Retirement Benefits" and is available on-line at the following link: http://www.osc.state.ct.us/agencies/forms/

Please note that completion of this form is solely required for the purpose of establishing an ARP member's eligibility for retiree health insurance and must be submitted along with Form CO-744, "Choice of Health Services After Retirement". To access their ARP funds at or after their retirement, members must make arrangements directly with ING, the State's third party administrator of ARP or TIAA-CREF, the former ARP administrator, if the member has retained funds on deposit with TIAA-CREF. Members may also access information concerning ARP at the following link: http://www6.ingretirementplans.com/SponsorExtranet/CTARP/ For funds on deposit with TIAA-CREF, members can call 1-800-842-2733 for information.
