Subject:	Forms Approval Process

I. INTRODUCTION

The purpose of this memo is to restate to all State agencies the approval process for requests to redesign or change Comptroller forms. The Comptroller?s Fiscal Policy Division has administrative authority for all Comptroller forms. Any changes to Comptroller forms that have not been approved by the Fiscal Policy Division will not be accepted.

II. PROCEDURES

Requests for changes to all Comptroller forms are to be forwarded to:
