Attention:	Chief Administrative and Fiscal Officers, Business Managers, and Personnel and Payroll Officers
Subject:	Request for Information Regarding State Employees in the Reserves of the Armed Forces of the United States

This Office is proud of the dedication and commitment of our fellow workers in this time of international crisis. Our obligation to those affected employees who are called to service in support of this crisis is to provide them with the comfort of knowing their benefits are in order.

Due to the likelihood of activation of employees in the military reserves and National Guard, it is necessary to supply our Office with the following information. The attached form is provided for your use.

Name of the employee;
Employee number;
Social Security Number;
Home address and telephone number;
Name, address and telephone number of any contact person for the employee;
Health Insurance coverage;
Branch of service;
Employing agency and contact person in that agency.
