Subject:	CO-210 Affidavit of Qualifying Wartime Service for Longevity Payments

I. GENERAL INFORMATION

This memorandum is a supplement to Comptroller Memoranda 2000-31 and 2000-31a. The attached form, CO-210, Affidavit of Qualifying Wartime Service, has been developed for use by state agencies to document employee claims of qualifying service when determining an employees eligibility for longevity payments.
